A family's tragic event--the death of two teenage boys in a car accident--is both the stimulus for a mother's abandonment of her husband and daughter and an ongoing thread weaving its way throughout the rest of this immense story (537 pages) told in three major parts.
Part 1 (1958) is the story of Marion and Ted Cole and their four-year-old daughter Ruth. Struggling to keep afloat in her grief-filled life, Marion is a beautiful, 39-year-old woman who, with her husband Ted, a hugely successful children's author/illustrator, lives an elegant life on Long Island. The focus of Part 1 is Marion's affair with Eddie, a 17-year-old hired by Ted to be his personal assistant but who turns out to be part babysitter to Ruth, and "companion" to Marion. This part of the story is sexy and comic, even as it is full of relentless grief.
Part 2 (1990) finds Ruth as a hugely successful novelist in her thirties. Her life is one long unending string of "bad" boyfriends, and one long question regarding how her mother could abandon her and why she fails to reappear. While in Amsterdam on a book tour, she comes up with the idea for a new book that takes her to the storefront prostitution district of the city, where her authorial curiosity and adventure is met with violence. In this section of the book she marries her agent, has a baby, and seems to be finding contentment for the first time in her life.
Part 3 (1995) occurs four years later, when Ruth as a 41-year-old widow and mother, falls in love. The story comes together finally with the reappearance of Marion Cole, now in her seventies and herself a moderately successful author who had been living quietly alone in Canada.

She writes of relationships starting and ending; her work experience as a nurse--the powerful "It Was My First Nursing Job," "From the Diary of a Clinic Nurse, Poland, 1945," and "Twenty-Four Week Preemie, Change of Shift"; and other miraculous everyday events of living with eyes wide open as exemplified in "The Brothers on the Trash Truck and my Near-Death Experience" and "Fever, Mood, and Crows."
Richard Kraft is about as burnt-out as a fifth-year resident in pediatric surgery can be. Overwhelmed by his stint in an inner-city, public hospital in Los Angeles, he seeks to hide from the misery of his patients by avoiding any personal connection with them. Then he meets twelve-year-old Joy, an Asian immigrant trying desperately to learn the puzzling ways of her new culture. She speaks words that trigger memories from Kraft's own childhood as the son of a U.S. agent in Joy's country, and he loses his distance.
He performs surgery on a life-threatening cancer in her leg, pulling back at the last minute in an unreasonable fear that he will hurt her if he cuts too deep. The implied result: incomplete excision of the cancer and a death sentence for the child he now tries, unsuccessfully to avoid. His avoidance is repeatedly foiled by Linda Espera, the physical therapist with whom he is falling in love and who will not let him abandon the emotional needs of any of the children in Joy's ward.
Dr. Terry McKechnie works in the emergency room in a Los Angeles hospital in the early 1990’s, and is having an affair with Virginia Lee, the new wife of an old friend of his from medical school. Virginia works with snakes. She is attracted to danger. She falls in love with Terry immediately after deciding to marry the reliable Rick, with his predictable dermatologist’s hours and habits.
Virginia is bitten by a rare snake and drives herself to Terry’s hospital. The drive is terrifyingly described, time seeming to move at two speeds at once, as Virginia sits stuck in traffic trying not to panic, the terse prose capturing her efforts at clarity even as the rapid effects of the venom begin to cloud her thoughts. Because she is allergic to horses, the antivenom, made of horse serum, cannot be given to her, and she begins to bleed. Unfortunately, she also has an extremely rare blood type.
As well as being a doctor, Terry is a "universal donor": his blood can be given to people with most other blood types without danger of rejection. His gift fails him at this point, however: Virginia must be given blood of her own type. One person has such blood: a psychopath on the run from the police whom Terry had previously allowed to escape. The novel’s plot culminates in Terry’s search for and encounter with the convict, in which he persuades him to give his blood (and, necessarily his freedom--he is arrested in the hospital) and Virginia survives.
Helen van Horne, the good doctor, has left her solitary practice as a physician in rural East Africa to become chief of the Department of Medicine at City Hospital. She is a tough, hard-as-nails woman who has given up the comforts of marriage, family, and human indiscretion for her profession. While the folks in South Bronx initially "hate" her because of her skin color, they soon learn that van Horne is a model of rectitude, dedication, and compassion. The Hispanic chief resident becomes her disciple.
Van Horne's yearning for human comfort and sexual gratification brings her into intimate contact with a lazy, irresponsible, but charming male student. Out of weakness, she tells the failing student that he will pass the rotation, but is later challenged by the chief resident, who has left her own lazy husband and dedicated herself to be "just like you." Van Horne realizes her own weakness, allows the failing grade to be recorded, then contemplates (but rejects) suicide.
Summary:A medical student at Lincoln Hospital in the south Bronx is called to the Emergency Room, where a gang war is going on. She plunges through fighting bodies in the waiting room to reach the treatment area, where she is enlisted to ride an ambulance with a critically injured and intubated patient. During the trip, the ambulance jerks, the tube comes out, and the student tries but fails to ventilate him with mouth-to-mouth respiration. At the other hospital, she is turned away from the cafeteria because her clothes are blood-soaked. The patient dies. Although she feels terrible, her friend Marie, a practical nurse, comforts her with a touch of ordinary love, a photo of her new grandchild.

The title of this volume is taken from William Carlos Williams's Patterson, where the spray over the falls on the Patterson River "brings in the rumors of separate worlds." In the Introduction Coles evokes his friendship with Williams and Williams's vision as the stimulus for this poetry.
The first section consists of poems evoking incidents and people from the author's childhood. The second section includes a number of militant poems from Coles's 1960's work with black children in the South, and later poems dealing with Nicaragua and Northern Ireland. The final section, entitled "On the Day Jesus Christ Was Born," is a set of Christmas poems evoking various times and places in the poet's life.
